The workshop covers everything from navigating local, state, and federal regulations to choosing between accepting insurance or private pay, and utilizing key applications, systems and processes that can assist with day to day tasks. You'll receive all the necessary New Client paperwork needed to get started. This includes lawyer reviewed and approved documents: Consent to Treat, Adult Intake Questionnaire, Child Intake Questionnaire, Private Insurance Cancellation Policy, Husky Insurance CAncellation Policy, Court Fees, Insurance Authorization, Credit Card Authorization, Release of Information, Phone Communication Policy, and Telehealth Consent Form. You'll also receive self-report symptom assessments that insurance companies are beginning to ask for.
